The Breeder must complete the LITTER SECTION and the BREEDER SECTION below. If the breeder does not own
the sire, the sire's owner must complete and sign the SIRE OWNERS SECTION below. In addition, if the sire is not
ERCA registered the owner of the sire must give the breeder a copy of the sire's registration papers to submit with this
Puppy Application Form. This form and accompanying paperwork MUST be submitted to ERCA at the address listed
below. Once this application is processed, the Pedigree will be mailed to you. If you have any questions feel free to
